Annual Reports and Documents

The Greater Western Area Health Service, provides high quality care to thousands of patients, every year.

In a small number of cases, adverse incidents do occur. As part of NSW Health's open disclosure policy, annual Root Cause Analysis investigations are undertaken into these significant incidents to help clinicians learn from adverse events and improve health services provided to our community.

In addition to RCA reports from The Greater Western Area Health Service, there is also a link to the Clinical Excellence Commission. The CEC was established in August 2004 as part of the NSW Patient Safety and Clinical Quality Program to ensure that patient safety and excellence in clinical care remain top priorities for the NSW Health system.
